This team includes:
Primary Care Nurses
Our Primary Care Nurses are registered nurses (RN's) whom offer care in chronic disease management (CDM) to help patients to manage chronic illness & improve their health. Such illness include, but are not limited to care in: diabetes, high blood pressure, high cholesterol, obesity & smoking. They provide individualized support, such as help for new diabetics starting insulin as well as facilitate group programs & workshops. Our associated Physician clinics are also supported with a LPN whom assists with small procedures & injection services (stich removal, ear syringing & intramuscular medication administration).
Prenatal Care
Prenatal Nurses provide prenatal care to patients and their families with a focus on education and holistic care. A team of skilled, experienced Physicians, as well as three Prenatal Nurses have committed to caring for maternity patients. Within the clinic patients have access to other members of the multidisciplinary team; such as a Dietitian and Therapists. All patients are referred at 24 weeks gestation to the delivering Physician of their choice. The prenatal team is eager to to meet with new and returning patients to provide the best prenatal experience possible! Appointments are available by booking or walk-in.
Women's Wellness & Teen Health Nurses
Our team of female nurses work together with two local female physicians to provide care to women and teens in the community. Services offered include: Cervical Screening (Pap Test), Breast Health, STI Education, Screening & Treatments, Birth Control Education & Options, Pregnancy Testing (with Immediate results), Fertility Testing, Menopause, Relationship Support, Domestic (or Dating) Abuse or Violence Issues & Self Esteem & Body Image Concerns. Appointments are available by booking or walk-in.
Foot Care
Our Footcare Nurse conducts a total foot health assessment for diabetic patients. This includes nail trimmings/filings, corn/callous reduction, and management of any fungal or ingrown toe nails. Foot health management services are provided Tuesday, Wednesday and Thursday. Visits are by appointment only and require a PCN Physician or CDM health professional referral.
Dietitian
Our Diettiian provides individual nutrition counselling and support to adults, teens and children.The dietitian primarily provides counselling to patients requiring support with chronic illnesses such as diabetes, heart disease, obesity and high blood pressure. Nutrition counselling is also available for other nutrition related concerns, including (but not limited to) food allergies/restrictions, gastrointestinal health, sports nutrition and general healthy eating.
Psychologist and Therapist
The PCN offers therapy to patients hoping to improve their personal and interpersonal functioning. Common topics for counselling include: issues related to stress management, adjustment to diagnosis/illness, caregiver burnout, relationship hardship, grief and loss, among others. Each member of our team has a unique set of skills. In order to match you with the most appropriate mental health professional, a brief telephone intake will be done before booking.
